RESUMO

Diffuse muscular involvement is common in hypothyroidism and ranges from asymptomatic elevation of creatine kinase levels to severe muscle weakness and rhabdomyolysis. We report a 74 years old male that developed a progressive muscle weakness without muscle pain, during the course of an hypothyroidism secondary to a silent thyroiditis. Laboratory showed a TSH of 149 uUI/ml, a thyroxin of 1.5 ug/dl a free thyroxin of less than 0.15 ng/dl and a creatine kinase of 4345 U/l. Treatment with levothyroxine rapidly reverted the muscular alterations.

RESUMO

Objective To observe the changes and significance of blood fat and myocardial enzymes in patients with hypothyroid myopathy before and after substitutional treatment.Methods The levels of TC, TG, LDL-C,AST, LDH, HBDH and CKMB were detected in 72 cases with hypothyroid myopathy patients before and after substitutional treatment and 60 healthy controls. Results The levels of TC,TG and LDL-C in hypothyroid myopathy patients before substitutional treatment were significantly higher than those in control group ( all P < 0. 05 ), and then significantly low after treatment,which had no significant difference with control group( all P >0. 05). The levels of AST,LDH,HBDH and CKMB in hypothyroid myopathy patients before substitutional treatment were significantly higher than those in control group( all P < 0. 05 ) ,and then significantly low after treatment,which had no significant difference with control group( all P >0. 05). Conclusion The levels of blood fat and myocardial enzymes were significantly increased in hypothyroid myopathy patients, and recovery to normal after substitutional treatment.

@# Objective To study the characteristics of hypothyroid myopathy in adults. Methods The clinical, biochemical, electromyographic, and pathologic characteristics of 19 patients with hypothyroid myopathy were analysed retrospectively. Results Proximal muscle weakness and elevation of muscle enzymes were the most common features. Other common clinical manifestations were myalgia, cramps, pseudomyotonia or stiffness after exercise. Muscular weakness with pseudohypertrophy was presented in some patients. Myoedema of temporal muscle was elicited by chewing continually. Hypothyroidism may be related to the pathogenesis of myasthenia gravis. Patients treated with levothyroxine achieved satisfactory outcomes. Hypothyroid myopathy is different from polymyositis in many aspects. Conclusion All patients presenting muscular weakness or elevation of muscle enzymes should be studied with their function of thyroid in order to avoid misdiagnosis.

RESUMO

Hypothyroidism may be accompanied by different rheumatic syndromes such as neuropathy. Arthropathy and myopathy. Sebere myopathy with high elevations of creatine kinase (CK) is rarely seen in hypothyroidism. Authors experinced a case of myopathy with hypothyroidism. Similar to rheumatoid arthritis or polymyositis like syndrome. We have treated with thyroid hormone replacement (Levothyroxine sedium 100ug/day) for 10 months. The initial serology resulted in : free T4 0.51ng/dl, TSH 110ulU/ml, LDH 465 units, and CK 4385 units. After tratment, we gained free T4 1.60ng/dl, TSH 0.14uIU/ml, LDH 110 units, and CK 80 utits and musculoskeletal symptoms are much improved We reprort this patient with a review of literatures.
